Pre-K Entrance Requirements

Minimum Immunizations Requirement

DPT-4 doses of DPT. If DPT was given, your physician must write a medical contraindication letter on his stationary. This letter will be kept in the student’s permanent health records.
Oral Polio – a minimum of 3 doses.
Measles, Mumps, Rubella or MMR-1 dose of MMR or of each component on or after the 1st. birthday.
Hib – age appropriate immunization.
Proof of Immunization – A copy of your child’s immunization records from your physician or local health board.
Tuberculin Test – A current TB test is not required, please ask your physician if they would like to update it.

A report of a physical examination done within one year of your child’s entrance to school.
